New Patients ($150.00)

If you are a new patient, you will need the following items for your appointment:

  • Valid state-issued photo ID
  • A copy of medical records with demonstrated history of a qualifying diagnosis
  • An active email address that you have access to
  • Valid form of payment for your appointment

Renewal Appointment ($100.00)

  • Annual visit
  • Log-in to your Ohio Medical Marijuana Registry account and activate your renewal

  • Veterans
    • Please bring valid military ID or DD214 showing the character of discharge
  • Indigent
    • Please bring proof of monthly disability income for the current year
  • SSI/SSDI
    • Please bring proof of status/award letter
  • Caregivers
    • There is no discount associated with having a caregiver, however, if you have difficulty leaving your home or require assistance from a caregiver, you may have a designated caregiver purchase products on your behalf. Caregivers will also need to bring a valid state ID, hold a separate email address, and register for a separate account (subject to $50.00 fee) with the Ohio Medical Marijuana Registry (caregivers cannot be recipients of Medical Marijuana themselves).

3 Steps to Obtain your Medical Marijuana Card

  1. Schedule an appointment with our certified medical marijuana physician by requesting an appointment.
  2. Bring your ID, medical records, and payment method for your appointment to your scheduled visit and complete your appointment with the physician.
  3. Once our physician determines that your are medically qualified for medical marijuana, they will issue your recommendation within 30 minutes so you can obtain treatment at the dispensary the same day.

Qualifying Conditions for a Medical Marijuana Card

According to regulation regarding medical marijuana in the state of Ohio, there are several conditions that may qualify you for a medical marijuana card. These conditions may include chronic illnesses, certain mental health conditions, and chronic injuries.

Currently, the following are considered qualifying medical conditions that may be treated with medical marijuana:

  • AIDS
  • amyotrophic lateral sclerosis (ALS)
  • Alzheimer’s disease
  • cachexia
  • cancer
  • chronic traumatic encephalopathy
  • Crohn’s disease
  • epilepsy or another seizure disorder
  • fibromyalgia
  • glaucoma
  • hepatitis C
  • Huntington’s disease
  • inflammatory bowel disease
  • multiple sclerosis
  • pain that is either chronic and severe or intractable
  • Parkinson’s disease
  • positive status for HIV
  • post-traumatic stress disorder
  • sickle cell anemia
  • Spasticity
  • spinal cord disease or injury
  • terminal illness
  • Tourette syndrome
  • traumatic brain injury
  • ulcerative colitis

In February 2021, the State Medical Board of Ohio determined the following conditions are considered covered by an existing qualifying condition:

  • arthritis
  • chronic migraines
  • complex region pain syndrome.
  • These conditions were determined to be covered by pain that is either chronic or intractable.

Once you are registered for the program, you are required to pay a fee of $50 ($25 if you receive SSI/SSDI or you are a veteran) to the State of Ohio Board of Pharmacy to activate your Ohio medical marijuana dispensary ID card.

Which Medical Marijuana Products to Use

Medical marijuana use varies among each patient. Our physician recommends keeping a journal to log products that you have used, how much of it you have used, what time/days you are using the products and how many times per day. This will help you determine which products are most beneficial to improving the symptoms of your qualifying condition. You should always start at the lowest possible dose and gradually increase the dose as needed to avoid any unwanted effects.
